| Stress Related Oral Disorders - A Review
D Nagabhushana, B Balaji Rao, GP Mamatha, Rajeshwari Annigeri, J Raviraj July-September 2004, 16(3):197-200The oral mucosa is highly reactive to psychological influences, and in some cases oral diseases may be a direct expression of emotions or conflicts, while in other instances, lesions of oral mucosa may be the indirect result of an emotional problem in terms of fundamental psychologic theory, the oral tissue have considerable significance. The mouth is directly or symbolically related to the major human instincts, passions and is charged with a high psychological potential.
The basic concept in psychosomatic medicine was first introduced clearly by SIGMOND FREUD, who used the term "conversion hysteria" in describing the reaction in which emotional conflicts are converted bodily or somatic symptoms.
When an emotion cannot be expressed normally or consciously, the individual may resolve this frustration by overt aggressive acts and gain relief in this manner however if the individual has built up tension that he does not relieve by "blowing off stream", the tension may be expressed subconsciously as a somatic symptom.
However, relatively few studies have been carried out on the relationship of emotional factors to diseases of the oral mucosa. So, here is an article which tries to briefly review the psychosomatic (stress related) disorders related to the oral cavity. |

| Diagnostic Imaging for Implants

The key to successful implant placement, its subsequent osseointegration and the final prosthetic rehabilitation is proper preoperative assessment. Diagnostic imaging plays an important role in the pre- and post-surgical evaluation process. Imaging is used to evaluate suitability of implant sites, aid in selection of appropriate implants, and finally evaluate implant placement and osseointegration. This article reviews the role of diagnostic imaging in the various phases and the advantages and limitations of the numerous imaging modalities. |

| Bilateral Masseter Muscle Hypertrophy : A Report of 2 Cases and Review of Literature
Subramaniam V Arun, Anurag Satpathy July-September 2004, 16(3):232-236Massetric hypertrophy is a benign increase in the size of the masseter muscle. It produces facial asymmetry and is an important finding in the differential diagnosis of similarly located entities. The aim of this article is to report 2 cases of bilateral massetric hypertrophy. The characteristics and diagnostic features of such changes, the specialized imaging modalities used for diagnosis and options for treatment are presented.
A proper diagnosis of this condition would avoid more aggressive and unwarranted therapy by an inexperienced clinician who may mistake it for a more serious pathologic condition. |

| Perioral Molluscum Contagiosum in a HIV Seropositive Patient - A Case Report

Molluscum Contagiosum Virus (MCV) is transmitted by person-to-person contact, sexual contact or by fomites. MC is seen in immunocompromised and HIV infected patients. In this article, we report a case of perioral molluscum contagiosum in a HIV seropositive patient along with the pathogenesis of the disease and various treatment modalities.

| Dimensional Distortion in Orthopantomogram
Vaishali Keluskar, Anilkumar Bhoweer July-September 2004, 16(3):245-247The Orthopantomogram is a laminographic type of radiograph, wherein the image suffers from a variable degree of horizontal, as well as vertical distortion. This study was carried out to know the amount of percentage distortion in the images of metallic spheres on the Orthopantomograph in different areas of both the jaws.
The study revealed that horizontal distortion was maximum in the mandibular midline followed by maxillary midline and was least in the maxillary premolar followed by mandibular premolar. The vertical distortion was maximum in the mandibular midline followed by maxillary midline and was least in the mandibular premolar followed by mandibular molar. |

| Need for a Vehicle in Topical Oral Medications
Prashant Nahar, Freny R Karjodkar July-September 2004, 16(3):214-222The Oral mucous membrane reacts to various forms of noxious stimuli and unknown etiological agents, manifesting distinct clinical forms on it's surface. Among the galaxy of diseases afflicting the oral mucosa and manifestating themselves in different forms, Lichen Planus (LP) and Recurrent Apthous (RAU) constitute two of the most commonly encountered debilitating vesiculo-erosive-ulcerative lesions.
The topical application of glucocorticoids is promoted in stomatological practise because it produces a high level of benefit with a minimal level of side effects when used cautiously.
However, attempts at the treatment of these lesions with these potent medications have frequently been hampered or delayed to the point of relative ineffectiveness due to the difficulty in maintaining these medications at the site of application for appreciable duration of time and quantity in oral cavity, mandatory for their optimum efficacy. This limitation has been sought to be overcome by combining a passive adhesive vehicle with the parent drug. |

| Non-Hodgkin's Lymphoma - A Case Report
Gowri P Bhandarkar, Leela Nagvar, G Savitha, TK Ramamurthy July-September 2004, 16(3):237-241Non-Hodgkin's lymphomas (NHL) constitute a diverse group of lymphoid neoplasm that differ in clinical manifestations, response to therapy and prognosis. Non-Hodgkin's lymphoma in the mouth, however is less common. When it does occur, it can develop in either osseous or soft tissues of the oral cavity, the most frequent sites being the tonsils, palate and buccal mucosa. Swelling, pain, paresthesia, anesthesia, ulceration and discoloration are common symptoms of oral lymphomas. Non-Hodgkin's lymphomas commonly affects the elderly and immunosuppressed with a slight predominance in males.

| Nucleolar Organizer Regions - A Tissue Marker in Oral Submucous Fibrosis Anita Balan, R Rajendran July-September 2004, 16(3):205-210Background: Oral Submucous Fibrosis (OSF) is a chronic insidious condition with a high risk for cancer development. Assessment of malignant potential of OSF through clinical or light microscopic tissue sections is not totally satisfactory. A tissue marker that will be useful in prognostication can benefit in its clinical management.
Method: Nucleolar organizer regions (NORs) are loops of ribosomal DNA found in the nucleolus that can transcribe for ribosomal RNA. The nonhistone nucleoproteins associated with NORs can be identified by silver staining (AgNORs). AgNOR staining of formalin fixed paraffin embedded biopsy specimens from 28 OSF patients and 10 normal persons was done. The relationship of mean AgNOR count to both the clinical stage and histological grades of disease was studied.
Results: There was significant progressive increase in pooled mean AgNOR counts with increasing grades of OSF. Mean AgNOR counts of OSF with coexisting carcinoma was significantly higher. When clinical staging was considered a wider range of mean AgNOR counts was seen.
Conclusion: AgNOR counts can be used as a simple and reproducible biological marker of impending malignancy. This could be treated as a advantage over conventional histopathology and / or clinical staging of the disease. |

This report presents 9 cases of complex and 1 case of compound odontoma, all of which incidental radiographic findigns. |

| Unusual Presentation of Adenomatoid Odontogenic Tumor - A Report of Two Cases J Rema, Anita Balan, Deepa Das July-September 2004, 16(3):228-231Adenomatoid Odontogenic Tumor (AOT) is a relatively uncommon odontogenic tumor Characterized by the formation of duct like structures by the epithelial component. According to the reports most of the cases occur in the second and third decades of life with only a few cases occurring in people over 50 years. Most often this tumor occurs in the maxillary anterior region with few cases occurring distal to premolar region. The most common radiographic appearance of adenomatoid odontogenic tumor is a sharply circumscribed unilocular radiolucecy associated with the crown of an unerupted maxillary anterior tooth especially canine. Radioopacities were reported in two thirds of the cases. Here we report two unusual presentations of adenomatoid odontogenic tumor. These cases are of particular interest because the site of occurrence, size of lesion and radiographic findings were not typical of adenomatoid odontogenic tumor.

| Primary Systemic Amyloidosis Presenting as Macroglossia
Karthikeya Patil, VG Mahima, B Shivanand Bagewadi July-September 2004, 16(3):189-196Amyloidosis is a rare disease of unknown origin characterised by extracellular deposition of fibrillar proteinaceous substance called amyloid having common morphological, structural and staining properties but variable protein composition. Oral manifestations have been reported in 39% of patients with amyloidosis and it manifests frequently as macroglossia.
A rare case of Primary Systemic Amyloidosis presenting as Macroglossia in a patient with no known antecedent or coexistent disease is being presented and discussed. |
